Who is Eligible for PCBDD Services?

Children under age 3 eligible for Service Coordination and Board funded Early Intervention services.
1. Children who have or are experiencing a developmental delay in one or more of the areas as measured by an evaluation tool and clinical opinion.
   a. Cognitive development
   b. Physical development, including vision, hearing and nutrition
   c. Communication development
   d. Social or emotional development
   e. Adaptive (self-help) development

Children under age 3 eligible for Service Coordination only.
2. Children who have a diagnosed physical or mental condition that has a high probability of resulting in a developmental delay or disability that is based on a written medical report.

Children at least 3 but under 6 years of age eligible for Service and Support Administration Services and Board funded supports.
 1. Children who have or are experiencing a developmental delay in at least two of the following areas
   a. Cognitive development
   b. Physical development, including vision, hearing and nutrition
   c. Communication development
   d. Social or emotional development
   e. Adaptive (self-help) development
 2. Children who have an established risk for a developmental delay.

Individuals at least 6 and older eligible for Service and Support Administration Services and Board funded supports.
 1. Individuals at least 6 and older must have:
   a. a substantial functional limitation in at least three of the following areas of major life activity, as appropriate for his age:
    i.   self-care
    ii.  receptive and expressive language
    iii. learning
    iv.  mobility
    v.  self-direction
    vi.  capacity for independent living
    vii. if the person is at least age sixteen, capacity for economic self-sufficiency
